can someone help me with an appeal letter to bc/bs of illinois,ppo?

I've been denied by bc/bs of illinois, ppo, twice. The first time was for the 6mth. diet. My pcp wrote a letter trying to cover that, but they didn't accept. I haven't recieved the actual letter yet, so I'm not certain what they've said this time. The rep. read it to me over the phone and it was several reasons. I need an example letter of appeal to go by if anyone has one or any suggestions at all would be helpful. Thanks and God bless. Kim
