Can someone help me with a letter of appeal for exclusion for a specific reason?

I was denied by my PPO for WLS. There is an exclusion in the policy stating that it wouldn't pay if you've had this done before. Well, i had a stomach stapling 21 years ago, but not the Gastric Bypass. Is there someone who can offer me some suggestions please!! You can email me at [email protected] so much!!!
