Seeking Others With Affinity healthplan through Medicaid

I am 25 Years old and I weigh 407lbs. I have been overweight my entire life. I never really made it that big of a deal until I had my two children. Now that I have these two wonderful people to watchover I need help badly. I would like to stay alive long enough to see my kids grow into happy, healthy adults, unfortunetly I don't see it happening that way unless I get this surgery done. If anyone has Affinity or any other healthplans through medicaid and you have already had the surgery can you please give my some suggestions as to what doctor to go to that accepts it. Thank You
