Has anyone change insurance after getting started on the approval process

I have already went to the surgeon and did all the pre-testing, but I cannot get letter of necessity. First MD said he would recommend because medicaid would not pay and I would end up in debt, second my GYN said he would recommend if I had a surgery that he wants but i refused, third MD refused said he doesn't know me, and finally today another MD said that it too hard to get medicaid approval so try this Meredia. is there anyone who could not get letter of recommendation and change to insurance that does not require a letter from MD. and does not have the pre existing condition clause. PLEASE HELP I'M DESPERATE!!!!
