If anyone can please help!!!! I've just been denied by BCBS need help with appeal!!!!

BCBS of Florida blue choice plan says my BMI has to have been 40 or higher for 2 years or longer and I need a medically supervised weight plan for a year. I am 5'4" and a half and have weighed at least 233 since 10-31-01. I have went to the doctor regularly since 97 and ever since I got off of Phen Phen have gained weight regularly. If anyone has any suggestions in what to put in my appeal letter please let me know.
