Cigna HMO - Out of network surgeon

Has anyone with Cigna Health Care HMO recieved an out of network refferal? I got my approval for RNY, but there is not a bariatic surgeon in network that performs the RNY WLS. Cigna says they will give an out of network ref. per my PCP request. Has this worked out for anyone? Any hidden charges?
