We have changed our insurance because couldn't get approval.

We have changed our insurance because we couldn't approval from Care Choices. We now have Blue Care Network but what are the guidelines to get approval can anyone tell me? What information do I need? I did call them couldn't get any information because I won't have the insurance until Jan. 1,2001. Please HELP.
