Need all info available to convince insurance company

My first appt is June 2nd. I am 5'4" 210 pds with additional medical problems. I know I am not "huge" but my health is at risk. I am worried that I am not "large" enough to qualify. I have to send info into dr before visit and want to make sure I answer "correctly". Any help would be appreciated so very much.
