State Standard Plan II

I am a state employee hoping to switch to the State Standard Plan II (BCBS of Wisconsin) so that I can get LAP-BAND surgery. Is there anyone out there who's attempted to get LAP-BAND through the STATE STANDARD PLAN II? If so, could you please email me at [email protected]?
