Does anyone know of a FedEx employee or employee dependent having wls?

My husband works for Federal Express. We are covered under Cigna POS insurance which is self-funded thru FedEx. I have been denied surgery, even tho I meet all medically necessary criteria. Cigna's criteria seems to differ from case to case. They told me I must have a 6 month diet within the last year, which I am complying with under the supervision of my PCP and the drug Meridia. My PCP monitors my excercise and I am on his recommendation following a VLCD. Now Cigna has said I must have a DIET doctor, a dietician, a SUPERVISED excercise program and be under the care of a behavior therapist. If I could afford all this, I wouldn't need insurance to cover wls, I could pay for it myself!!! I would like to hear of anyone either a FedEx employee or dependent who has successfully had this surgery, and had the company's insurance coverage pay. Or, any help to get this approved would be greatly appreciated.
