Has anyone used BC/BS of Indiana PPN?

I have never head of the PPN network until today. I got a lot of great information, but they failed to send me the list of exclusions (which they said they will send out today). Has anyone used the PPN before? If so, is there an exclusion against WLS? It's an individual/family policy. Thanks!
