CIGNA rejected me please help me get all my ducks in a row to resubmit

CIGNA rejected my surgery saying it is not a covered benefit. Also the doctor is not in my local coverage area. I will have to use a doctor that does open rny. I have an appointment for consultation on June 14th. I want to have all the documentation and arguement paperwork together when I go so it can be submitted and I won't get another rejection. I have sleep apnea, highblood pressure, degenerating carlidge in my knees, and a BMI of 63. What were they thinking. I have tried to read everything on the website but I'm not sure what will really turn this thing around. Any and all help will be appreciated. And I live in Florida Thanks
