Samples of letter

Help! I am so nervous. I saw my Primary doctor on Friday (8/20/99). He wanted a letter describing why I want this surgery..etc...my diet history...I am just so excited. I sat down to write and my mind went blank. I made my first appt. with the surgeon on Sept. 14..anticipating...my doctor's approval. Any thing you can send will be helpful! Thank you! Karmen
