Has anyone had the Fobi Pouch procedure approved through Cigna Healthcare?

I have Cigna Healthcare PPO insurance through Concentra Managed Care, Inc. I would like to get the Fobi Pouch procedure done and would like to know if anyone has gone through Cigna and if they encountered any problems. And how much the insurance covered. And if there are any costs that have to be paid upfront for this procedure. Thanks.
