I am in dire need of a revision, I have no insurance is there a dr. out there to help

I have no insurance and need a revision to have my clamp replaced. I weighed 409 pounds 14 years ago. I lost down to 180 pounds and had to have the clamp removed due to it keep collasping. I am back up to 320 pounds. Is there a dr. who would do a charity case due to my health. Please help me. I am from Indiana and am willing to travel for HELP.
