Question about insurance and Qualification in KY

Hi. Has anyone here that has had the surgery in Ky with either Dr. Rodriguez or Dr. Allen had issues with medicaid? Can you give me a heads up to what they have to have before approval. Do you have to show proof of dr. supervised diets, etc...I went to a Dr. in Campbellsville about 8 years ago that is now no longer in business and he had me on fastines (diet pill) I dont know how I would get records of that with him not being there anymore...so I was just wondering if Medicaid was hard to get approval with and what they want.
