Can I get approval without my file from my doctor?

My doctors original group, that I saw her with, no longer has my file. My doctor also stated that she no longer has my file. They both say they sent my records to the hospital records dept. The hospital records dept. says they don't have any records from my doctor. I have a fax from each of these, basically showing that they say the other person has it. I need to prove my doctor supervised diets to my insurance company. What are the chances that I will be approved. (Due to this not being my fault.)
