please name a few for me

I have had some news from my pcp (who is NOT wls friendly) about boarderline comorbilities(cholesterol and low hemoglobin.) I have incontanance, asthma, and joint and back pain to name a few. can someone please send me a list of comorbids so I know what to write about to my ins. co BCBS of SC point of service. I don't have the support of my pcp so I am doing this by ear. please send as much info as possible. THANKS!
