Did your health insurance rates go down after you lost the weight?

I pay more for my insurance because of my weight, so I was wondering if the rates decrease once the weight is lost. Also, I was wondering if anyone had to switch insurance companies after surgery and if so were you denied coverage because you had had the wls? If you are denied is it just for a certain number of years or what? Thanks.
