I need your help, I have United Healthcare POS ILLINOIS

I work at Wells Fargo, Illinois. I have United Healthcare POS. I have been trying to have this done for 2 years. I keep on getting denied by UHC POS ILLINOIS. They keep giving me the wrong address/ fax number to send my paperwork. Ia m so tired of all of this. It's Driving me CRAZY! Does anyone have any advice?
