help with out of pocket expense ,medicare and ins approval

I need to know how much out of pocket expense i can expect. I am on disability (S.S.)because of my extreme weight and I have Medicade Part A and Part B. Also does anyone know if The people at the Baylor Weight Loss Surgery Center help you with prior insurance approval and do they have a finance program to let you pay out any uncovered expenses.I have very few resourses and receive a very small income from Social Security.
