Has anyone been approved by BC/BS ( General Motors)Preferred Choice Plan?

Hello Everyone! I live in North Carolina and have BC/BS of Michigan through General Motors ( Preferred Choice Plan)/Traditional. I have my first appointment in December for the Lap Band and I was wondering if anyone with my insurance has been approved for the Lap Band Procedure? I have called BC/BS but nobody will tell me anything unless I have the insurance code and the nurse at my doctor told me they cannot give it to me until I've had my appointment. Dianna
