Has anyone had problems with Pacificare approving a TT...?

I'd like to know if anyone has had any problems being approved for a TT, Abdominoplasty, or breast lift with Pacificare. Do they cover any or all of these? What about brachioplasty and an inner thigh lift? Are these covered for redundancy and/or for medical necessity...such as for rashes, infections, etc...?
