Can someone help me get some sample letters from PCP's to insurance?

I was hoping I could see some samples. I am 25 and only have two comorbs (GERD and hypertension) well, and Adema too, but I weigh 340 and have a BMI of 56... i am told I need a PCP to write a letter of medical necessity. My PCP is not for or against the surgery and said he would help me any way he could to make me happy. Can you give me some sample letters that are really good that I could write with my medical stuff and have him sign?
