I am having trouble getting insurance to pay. Any suggestions?

I had WLS in July 2001. BC/BS of Alabama paid the hospital and doctor bill, but not the anesthesiologist. I called and the claim was re-submitted, but as of today, it has not been paid. Does this happen often? I asked how can you have surgery with anesthesia, the two go together.
