Need help with an appeal letter for Aetna QPOS

I just got my first denial letter from Aetna and need some ideas for an appeal letter. I have already gotten copies of everything from my doctors office and plan to have my husband take some photos of me to send in with my appeal letter. What I need is help actually composing the letter. Any ideas would be appreciated.
