I was denied when I went for my pre-ops 1 wk before surgery after being approved 3

weeks earlier, due to a written exclusion by my employer, which happens to be a hospital. Has this happened to anyone? Any reconmendations welcomed, when do I start an appeal? Is it worth it? I felt so foolish when I had requested the time off and even had an approval number.
