Can my doctor write a referal for WLS?

I have an appointment with my PCP on monday so that I can convince him to write up a referal.I have already done my psyc eval, and have met with various nutritionists.My 1st appt with the surgeon is on the 30th and I need a referal from my pcp.Should I try to get him to write the referal for the surgeon,or should he submit one for the surgery??? please help! Thank You.
