How do I get approval for the sleeve from medicaid.

Emory MidTown has approval from Medicaid for the sleeve, but Atlanta Medical told me Medicaid has not approved it. I would get it done at Emory but the 600.00 co-pay is not in my budget. How do I ask medicaid about the approval myself?
