can anyone help me with a strong appeal letter to united health?

i was wondering if anyone could help me with a strong appeal letter? i have united health choice plus hmo and was denied because of an exclusion. my pcp and surgeon are more than willing to write them on my behalf. any help is greatly appreciated!
