I have Cigna POS how hard is it to get approved?

I have Cigna POS and I called and they said it is covered but only a medical necessity. I asked if there was any exclusions from my employer and they said "no". I have dieted off and one for several years but never with a doctor's assistance (too expense). So now I have an appointment with a PCP to get the 6 months of diet program that they told me I need to have. Any assistance from anyone that has been down this road would be great.
