has any one had this happen to you and what was the out come?

i have already been approved for sugery i have had my psych eval already so i thought smooth sailing after this well.......my dr put in for an approval for an upper G I and it was denied..now this is mandatory for my dr and i believe alot of others ...how could this be...has anyone had this happen to them? and what was the out come? thanks i am soooo anxious now so worried HELP!!!! (health net/phyc associt in s calif)
