Medically supervised diet? What is involved with that?

My BMI is 39.75 and I have 2 co-morbidities. What is involved with the 6 month supervised diet? What if I lose 20 lbs and my cholesterol and bp drop due to meds and weight loss? Do I just make sure I don't lose any weight during the 6 months?
