Anyone with Medicaid-Carolina Access,please help??

I have a similar question to one that has been asked,but I don't seem to find my answer. Anyone with medicaid-carolina access,please help me. I need to know more about this 6 month diet evaluation to be approved through medicaid. What did it involve?? My PCP hasn't had anyone to go through this surgery before,and doesn't know exactly what is required of him to put in this report. I am on my last pre-op tests,and don't want anything to go wrong. PLease help!!!! Any suggestions will be greatly appreciated.Thanks in advance.
