My doctor told me to write my own 12 month diet attemps!

my pcp told me to write the letter with all the info,bring it to his office & he would have his secretary rewrite it on company letter head and sign it.Sounds great right only problem is ive tried so many things i dont remember all of them and i absolutely have no clue how to word the letter.Does anyone have any suggestions or tips?
