Insurance from Samsung Semiconductor

Does anyone working at Samsung in Austin, TX have health insurance that paid for Gastric Bypass? If so can you tell me what is the pre-requirements. I am asking for my daughter, she has asked her insurance they have not as yet sent her the papers on it. What doctor did you use? Thanks for the information.
