Does indiana medicade approve even without documented diets by your doctor?

I have done a number of diets, but i know for sure i havent been on one for more than a eight months. Close to it but not exactly. So what i want to know is what do they do if you havent been on a diet for a long period of time? Do they still approve it or not? I think i am on the right track because i have sent in everything that i have and more, if you have something to add please help any information helps. THANKS!
