bcbsnc deductible

Hi guys I have just started the process for the lap band. I contacted my insurance company and they said that bariatric surgery is covered with per-approval in my plan. Then she told me that I would have to pay a $2,500 dollar deductible. I'm wondering if I have to pay this up front or if it will be billed to me? If anyone has the same ins. or ran across this issue please answer my question. Thanks
