Has anyone been denied by their Insurance Carrier?

I ws recently scheduled for surgery, Gastric Bypass, on July 16, 2012; the surgeons office called woth the time of surgery, on Friday July 13; later the same day, they called me nack an said they were having a problem with my Insurance; my Insurance subsequently denied my surgey, saying that the intended hospital was not an approved Center for Excellence. Has anyone had this problem, and how do I fight it?
