I need to find 3 medically supervised weight loss programs

I just found out today that I have to have 3 medically supervised weight loss programs as required by my insurance. They also require they be in the last 5 years with at least weigh-ins bi weekly with progress notes from the doctor or nurse. they also can't be weigh-ins from a ym or a group like weight watchers (which I do have). So now it's like starting from new. Plus each time has to be 12 weeks long. So the earlist my surgeon with summit the paperwork to the insurance will possibly be 9 months to a year. I need to know of 3 programs, medically suopervised, that won't cost an outragous amount.
