who submits your official weight to the insurance company?

I saw my pcp last week and I weighed 207, at the weight loss spacialist(helping me through the surgery journey)I was 203, and at the nutritionist I was 204.I have not met with the sugeon yet,according to the nutritionist my bmi is at 39.8 which means I am 1 lb away to qualify for for lap band with no comorbities,so what should I do??
