very severe constipation stool softeners do not help anymore

my bowels do not move unless i take an enema about once a week. Then I have a huge bm. I would feel better and lose weight easier if I could be regular at least daily. any suggestions had my surgery 7 years ago have put on 20 pounds trying to get this off plus 18 more. Thanks for your help.
