RNY revision?

I feel like a total falure I had RNY surgery in 2008 and have gaind almost all my weight back I dont understand why I hardly eat, I am considering a revision does any one have any comments or recomendations and does any one know how difficult it is to get this aproved by the same insurance that aproved you the first time ? I am currently on a wt loss plan and exercise daily but I can not get it off! HELP!!!
