I have a question about my insurance company and surgeon

Hi, I have Healthnet Insurance and needed 6 months of consecutive doctors visit. I have one more left and i'm getting nervous I may be denied. I have gained weight at every visit. I'm hoping to have surgery in November with Dr Iannace and Capella from Hackensack Hospital in NJ. Has anyone had experience with my insurance company or my surgeons? Thanks
