Questions and Answers for OHIP Form in Canada
I was just reviewing the paperwork that my doctor filled out and on the On the OHIP form in regards to the question: Is treatment required out of Canada to avoid a delay in obtaining treatment in Ontario that would a) result in death b) result in medically significant irreversible tissue damage. He answered no to (a) but yes to (b) He then stated treatment is required asap as the result would be increased cardiovascular risk and respitory problems. Do you think this is going to be a problem in regards to OHIP coverage?? Originally the other woman I spoke to said that the answer to (a) should be yes?? Please help...I am now concerned OHIP may not approve it based on this!! Do you know anyone that could help me answer this?? Any help would be appreciated! My Doctor did write me a letter fully supporting me having the surgery in the United States. Not sure if this makes a difference. Thanks so much, Georgia
