i am a bmi of 40, what qualifies you for lap-banding?

i have heart problem and am 55 yrs old. i just got insurance that will cover it and am waiting to schedule my 1st appt. what does the bmi have to be to qualify to get the surgery done. i am so afraid i will go thru all this and the doctor will tell me no.
