list of pre-op questions needed ASAP! Lost my list

HELP a pre-op NEED pre-op QUESTiONS!!!!!!! Panic setting in my 1:1 is tomorrow someone had posted a great list of pre-op questions. I thought I had saved the list but I have not HELP ALlof a sudden i feel in a panic, cannot think of any????????....
