Should I be concerned about my consult?

I have my first consult at Palmetto Health Baptist on 4/5/09 at 11:30 I have been waiting for this appointment for over a year now due to have SC medicaid. On 4/29/09 a few days before my consult a lady called me from baptist but I didn't get her name. She asked "was I still interested and was I gonna keep my consult date" INsured her yes then she asked "did I have medicaid?" I replied yes she then goes to say that I needed another paper to fill out and that if I didn't have it they would turn me around at the door. Well I've been waiting for this paper in the mail but no paper what should I do? My appointment is in the morning at 11:30am. I told her that my Dr.office faxed over the referral already. Could someone please help me so that I'll have all that I need for my consult? I'm sure it would be very disappointing to get there only to be rejected.
