I have United Healthcare Choice Plus and want to know what they need?

I have United Healthcare Chouce Plus Insurance. I tried calling them but cannot get the answer I am searching for. I am wondering if anyone else who has this insurance can tell me how long it took for them to get approved and what the insurance needed from them? I am 5'8" and weigh 298 my BMI is 45.3. I do not have any major problems like diabetes, or high blood pressure, or even high cholestrol but all of these things run in my family. I do have arthritis in my back due to my weight, and I suffer from headaches everyday. If you have any information you can e-mail me or write a reply thank you so much for your help. Susan
