How many visits did you have for your Psych evaluation?

I recently scheduled my insurance required psych evaluation. When I got the confirmation from insurance in the mail, it shows that they have approved up to 4 visits for this evaluation. One for the initial consult, one for testing, one for follow-up and one for the doc to use for her "evaluation". Is this common? How many times did you have to go back? I have Ohio Med PPO.
